First Visit Information
At Cornea Associates of Texas, our physicians and team are dedicated to making sure your first visit to our office is as easy and productive as possible. The following information will help you prepare for your visit.
Scheduling your first appointment:
Appointments may be scheduled in either our Dallas, Plano or Fort Worth locations by calling our office during regular business hours (Monday - Thursday 8:30-5:00 and Fridays 8:30 - 4:00). We ask that you please have your current insurance information available when scheduling your appointment.
Preparing for your first appointment:
Please plan to arrive at our office by your scheduled appointment time. If you have not filled out your patient registration forms, we ask that you arrive 10 minutes prior to your scheduled appointment time. To assist in your appointment planning, please allow 1 1/2 to 2 hours for your appointment. Some appointments may require a dilated exam. It is best to arrange for someone to drive you home if your eyes are dilated. Certain tests performed in our office require you to remove your contact lenses in order to obtain the most accurate results. If you are able to function in your glasses, it is preferable that you remove your contacts for two weeks prior to your appointment.
Items needed for your first appointment:
- A valid picture ID
- Your current insurance card(s)
- Any necessary insurance referrals
- Prior medical records related to your current eye care needs
- Completed registration forms
- A current list of all medications you take, including all eye medications
- Your most recent eye glasses and/or contact lenses
Our practice is a sub-specialty ophthalmology practice, dedicated to the treatment of the cornea, refractive surgery such as CustomVue iLASIK and anterior segment procedures like cataract surgery. Patients can be referred to our practice through their regular eye care providers, in which case your most recent records from their practice will be extremely helpful. Patients can also be referred by a friend or family member - we accept and sincerely appreciate these referrals. Please note that due to the sub-specialty nature of our practice, we are not able to perform routine eye examinations for the purpose of obtaining glasses or contact lenses.
